Research and Academia

Objective: We are able to provide an opportunity to join the established clinical research team at The Department of Anaesthesia, Royal Victoria Infirmary and are keen for a trainee to be actively involved in the research studies that we support. This will afford the opportunity to develop an interest in research, research methodology, recruitment and organisation of local clinical trials and NIHR portfolio studies. The trainee will be accepted as an active researcher within our departmental fields of research.

Supervisors: Dr R Sinclair and Dr D Saunders.

Duration: 12 months, August to August.

Applicants / Advanced Trainees are required to meet a minimum set of training targets before commencing this training module. These are outlined in our person specification. Where there are a number of trainees interested in this post we will interview early to select a trainee.

Clinical Duties: the advanced research trainee will join the other trainees rotating to the RVI and benefit from the same clinical training opportunities. Clinical experience will be reduced by the time allocated each week to allow dedicated research. A motivated and organised trainee will be able to balance these opportunities to their advantage although it will not be possible to do an ITU training block during this year. Good planning and list allocation should allow the trainee to complete a second, clinical advanced training module during this year (for example: major surgery, day surgery). These could compliment the research module if planned at the start of the year.

On Call: the trainee will take part in the PINC and ORANGE (senior) on call rotas at RVI, with the same intensity as all other trainees.

Research Duties: one day a week of clinical time will be timetabled for research on a flexible basis, to fit around on call, service and other educational requirements. The personal learning plan will be tailored to reflect the current priorities of the supervising research team, ongoing work and, potentially, new opportunities. We envisage a ‘rolling’ programme of work evolving such that there is succession planning between consecutive trainees and completion of projects. The research will remain within the fields of interest of the permanent team. We have an established programme of research and new proposals within the department at RVI: these are based within emergency anaesthesia, perioperative medicine, oncology and major surgery. There are other active areas of research in the department where it may be possible to be involved if desired (e.g. regional or paediatric anaesthesia). We envisage the trainee becoming involved in the day to day running of our core studies: recruiting, data collection, data analysis and academic writing. The trainee will also help to deliver NIHR portfolio studies within the department.

Higher competencies: We anticipate that the trainee will work towards achieving higher and/or advanced level competencies in the Academic and Research domains within Annex G of the RCOA curriculum.

Research/Major Surgery – Two Advanced Training posts:

Exciting opportunity is available for trainees within the Department of Anaesthesia at South Tees Hospitals NHS Foundation Trust.  In collaboration with the Northern School of Anaesthesia we have created 2 Advanced training positions combining clinical work in ‘Anaesthesia for major surgery’ with research opportunities within the department.

The intention of this attachment is to provide both a year of clinical training that can be mapped to the RCOA advanced curriculum, and the opportunity to participate in research/academic projects as a senior trainee. The post is designed to incorporate teaching and solo lists in complex major surgeries, commitment to out-of-hours service provision and timetabled academic/research sessions.

Clinical training
The year is divided into 4 blocks of 3 months designed to fulfil the core learning outcomes for Advanced training:

  1. Anaesthesia to a wide range of surgical cases demonstrating understanding of the problems encountered
  2. Show the decision making and organisational skills required of an anaesthetist to manage busy operating sessions that involve major surgery and ensure the care that is delivered is safe and timely
  3. Assist colleagues in decisions about the suitability of surgery in difficult situations
  4. Provide teaching to less experienced colleagues of all grades

Major Abdominal Surgery Cystectomy, major colorectal surgery, Upper GI, Bariatrics and Gynae oncology WBAs as per 2010 curriculum (1x ALMAT)
Major Head and Neck Surgery Major Maxillofacial, ENT and Plastic reconstructive head and neck procedures WBAs as per 2010 curriculum (1x A-CEX, ALMAT, DOPS)
Option block This may be dedicated to academic/research or may involve further exposure to the subspecialty modules above.

Other possible modules on offer include Neurosurgery, Trauma & Orthopaedics, Day case surgery, Paediatrics, acute pain, regional anaesthesia, Human factors

WBAs as per 2010 curriculum if further clinical attachment undertaken.

The clinical expectation on which competence will be assessed is that the trainee will complete a minimum of 10 sessions in each subspecialty module of clinical work, allowing a minimum of 10 sessions per 3 months to be spent in academic/research activities.

Research opportunities
The department has an expanding research base with several exciting trials. These trials encompass research in both anaesthesia and critical care. This includes involvement in an exciting multi-centre national study assessing the benefits of preoperative exercise training prior to major vascular surgery. Other examples include trauma research with the Ministry of Defence and cutting-edge studies involving pre-operative assessment and cardiopulmonary exercise testing. The team comprises a number of research active consultants, 1 academic foundation programme doctor, a critical care research fellow and 2.3 FTE research nurses. The group has developed strong links to local universities with several collaborative on-going projects.

The previous ‘research fellowship’ position demonstrated an excellent track record facilitating several international/national presentations (8 prize winners) and peer-reviewed publications over the last 8 years. Previous research experience is not essential. The successful individuals will have a fantastic opportunity to be involved with on-going projects in addition to developing research ideas of their own.

An agreed research output will be set at the beginning of the post for the successful individuals.
